Support Coordination
Our coordinators help make an NDIS plan work in everyday life — explaining what the plan funds, finding suitable providers and building skills so decisions stay with the participant.

What's included
- Explaining the plan in plain language and mapping the budgets
- Searching for and comparing providers, and reviewing service agreements
- Coordinating mainstream services such as health, housing and education
- Gathering evidence and reports ahead of a plan reassessment
- Support to attend appointments and to self-advocate
What's not included
- Deciding on the participant's behalf — decisions stay with the participant or their authorised representative
- Specialist Support Coordination for complex, high-risk situations
- Specialist behaviour support assessment or the implementation of restrictive practices
- Clinical, nursing or high-intensity health procedures
- Emergency, crisis or after-hours response — call 000 in an emergency
How we personalise this support
We start with an unhurried conversation about goals, culture, communication preferences and risks, then agree how often we meet, who else is involved and how progress is recorded.
Who delivers this support
- NDIS Worker Screening where legally required for the role
- Working knowledge of the NDIS Code of Conduct and Practice Standards
- Experience in coordination, case management or a related human-services field

Funding and costs
Prices depend on the service, time, location and staffing, in line with current NDIS pricing rules. We explain proposed rates and any travel or cancellation charges before support begins, and include agreed costs in your service agreement.
