Every referral receives a written acknowledgement with a reference number and a named contact.

Intake within 48 hours

We complete intake, confirm capacity and provide an honest start date — including if we can't take the referral.

Reporting you can use

Progress notes, goal-linked reporting and plan review summaries written in plain English, delivered on schedule.

Escalation that works

A direct escalation contact for urgent participant matters, incidents and risk changes — no call centre queues.

Referral checklist

Have these ready and the referral takes about eight minutes.

  • Participant name, date of birth and NDIS number
  • Plan dates, plan management type and relevant budget lines
  • Goals and requested supports
  • Risk, behaviour support and medical information
  • Consent to share information with IDIM Care
  • Any current provider and handover details

How we keep you in the loop

Predictable communication means fewer chase-up calls for you and better outcomes for participants.

Weekly during onboarding

A short written update each week for the first month covering worker matching, attendance and early goal progress.

Monthly once established

Progress notes summarised monthly, plus immediate contact for incidents, risk changes or unplanned absences.

Plan review ready

Goal-linked reporting prepared ahead of plan reviews, with evidence of outcomes and recommended supports.
